Activate - Heart and Stroke Program

The Heart and Stroke Foundation of Canada is excited to announce a roll out of a FREE program, called Activate, to people of Toronto and area. The program is aimed to help those that have borderline high blood pressure lower it through advice, activity and support. As of January 14, 2019, the program has been launched to the Oakville area and they are currently recruiting participants.


What is the program?

The program is called Activate. It is a lifestyle-change program helping people with elevated blood pressure prevent the onset of high blood pressure. If the top number of your blood pressure is higher than 120, you could be at greater risk of developing high blood pressure, an important risk factor in heart disease and stroke.


Who qualifies for the program?

  • you are over 40 years old

  • you don’t have diabetes

  • you are not on blood pressure medication

  • you have systolic blood pressure between 121 and 139 (the top number). Blood pressure will be measured at your scheduled appointment.


If you qualify, the program consists of a number of support systems to assist participants:

  • an online platform to communicate with participants

  • a personal coach

  • a free 2 month membership to the YMCA

  • a consultation with an in store Loblaws dietitian

  • earn PC Optimum points
